Complete Student Health Requirements for 24-25 School Year
Complete Student Health Requirements by August 14th
- What:
- IMMUNIZATIONS: Per law, your child needs to be updated with the required immunizations for school attendance or have an exemption in place by the start of the school year.
- MEDICATIONS at Denver Online: If a student takes medication, please complete the Student Medications Form. This is especially important for any student attending a field trip and/or any in-person event or activity. Parents must sign and date any forms after health care provider completes the form. Students can self-carry epipens, inhalers, and some medications if self-carry is checked on the paperwork by the health care provider. The school nurse will then review the paperwork and complete a self carry contract with the parent and student.
- Review How Sick is Too Sick?
- Who: All students need to have updated immunizations or an exemption form completed. Medication forms must be completed for students taking medication.
- When: All documentation should be completed and submitted by August 16th

- Other Resources:
- DPS Medicaid: Provide free or low-cost health insurance, including Health First Colorado (Medicaid) and CHP+ and SNAP food assistance.
- Denver Health School Based Health Centers provide medical, mental health, dental and behavioral health care to any DPS student and their siblings at NO CHARGE.
