Complete Student Health Requirements for 23-24 School Year
What: Per law, your child needs to be updated with the required immunizations for school attendance or have an exemption in place by the start of the school year. If a student takes medication, please complete Medication Forms here. Students will need medication for field trips and/or in-person activities. Students can self-carry epipens and inhalers if that is checked on the paperwork by the health care provider. Parents must sign and date after health care provider completes the form. The school nurse will then complete a self carry contract with the parent and student.
Who: All students need to have updated immunizations or an exemption form completed. Medication forms must be completed for students taking medication.
When: All documentation should be completed and submitted to Denver Online’s Office Support, Veronica Martinez (vmarti4@dpsk12.net), by August 14th, 2023.
